Use this guide to prepare and ask informed questions. VA decides claims under its rules; an accredited representative can advise you about your individual situation.

Start with the request VA actually sent

A letter asking for private treatment records needs a more specific response than another general evidence checklist. Put the letter beside you and identify each provider, treatment period, or missing authorization it mentions. Your goal is to connect every requested item with a next action and a way to confirm that action happened.

Write down the response date shown in your notice. VA may decide a standard claim before the full evidence-submission year has elapsed, so do not treat a general reference to one year as permission to ignore the letter. If you cannot meet the stated date or do not understand the request, contact VA or your accredited representative promptly; asking for help does not itself extend a deadline.

  • Notice date and stated response date: ______.
  • Exact records, providers, or forms requested: ______.
  • Question that needs clarification, and who you contacted: ______.

Choose how each provider's records will reach VA

If you want VA to request private records, VA Form 21-4142 provides authorization to obtain them. VA Form 21-4142a identifies the providers and treatment dates. These forms serve different purposes: permission alone does not give VA a usable address or tell it which period of care you mean.

The current 21-4142 instructions say the authorization is not needed when you have already supplied the private records or intend to obtain them yourself. It is also not needed to obtain VA medical records. If your notice requests a form despite records you believe you already sent, compare the provider and dates before assuming the request is a duplicate. Clarify any remaining mismatch with VA.

Choose one route for each missing set: authorize VA to request it, or obtain and submit it yourself. You may have different routes for different offices. Record the choice so that an authorization receipt is not mistaken for proof that the treatment records arrived.

Check the office, dates, and signature before sending

Open the current forms through VA's form pages rather than an old saved attachment. Both form pages list an August 2024 revision when checked for this guide. Read the instructions on the version you use; VA also offers an online authorization option.

For each provider, confirm the name of the office or facility that holds the records, its complete mailing address, the condition treated, and the requested treatment dates. The 21-4142a fields ask for those details. A doctor's name without the relevant office address can leave a request incomplete. If a practice moved or closed, ask who now holds its records and record what you learn.

Check that the authorization has the required signature and date. If another person signs for the claimant, follow the form's instructions about authority and relationship. A completed provider list does not replace the signed authorization when VA is being asked to obtain the records.

Copy this register once for each provider

Keep this register with your own claim papers. It is a personal organizing aid, not an additional VA form, and this page does not collect or save your entries. Use enough detail to distinguish the requested records without copying medical narratives into the register.

Keep separate entries when different facilities or date ranges need different follow-up. For example, confirming an authorization for an orthopedic office does not establish that a hospital's imaging records were received. This is an illustration of record tracking, not a claim outcome.

  • Provider or records custodian and verified address: ______.
  • Treatment period and record set requested by VA: ______.
  • Route chosen — VA requests with authorization, or I submit the records: ______.
  • Authorization sent, if needed — date, method, and receipt location: ______.
  • Provider contact — date, person or department, and response: ______.
  • Records actually supplied — provider, dates covered, file name or page range: ______.
  • Submission confirmation — date, channel, and receipt location: ______.
  • Still missing, next action, and next check date before the notice deadline: ______.

Track a missing record differently from a missing release

VA's duty-to-assist guidance describes help obtaining evidence for initial and Supplemental Claims, including private records when VA has permission. It says VA will follow up on a private-records request and tell you if it cannot get the records. The same duty-to-assist process does not apply to gathering new evidence during a Board appeal.

For a standard disability claim, VA also explains that you remain responsible for making sure it receives requested records outside a federal agency. If a provider refuses, charges a fee, or cannot supply the records, VA may ask you to try to obtain them. VA does not pay those fees or postage.

Use the register to distinguish what went wrong: no signed release, an incorrect address, only part of the treatment period supplied, or a custodian that says the records are unavailable. Save the office's response and tell VA what you could and could not obtain. Do not mark an item complete merely because a request was made.

Use the right submission route and keep the confirmation

For evidence supporting a pending disability claim, VA directs you to its claim-status tool. Its supporting-evidence page also explains QuickSubmit for other documents and for decision-review or appeal documents. Those routes do not change which evidence a particular review option allows. Match the document to your current notice and claim or review type before sending it.

Use the official authorization tool or the current form's submission instructions when sending a release. If mailing disability-compensation correspondence, check the compensation destination on the current instructions; do not use an address for a different benefit or send the completed form to a survey-feedback email address.

Save a copy of what you sent and the available submission confirmation. Then update the matching provider entry. A successful upload establishes a submission step; it does not establish that VA found every requested page sufficient or that the claim will be approved. If the listed files or dates do not match your submission, follow up with VA or your accredited representative.

Common Questions

Does signing Form 21-4142 mean VA has my treatment records?

No. It authorizes a request. Track the authorization separately from the records themselves, including the provider and treatment period actually supplied.

What if I already sent the records mentioned in the letter?

Compare the letter's provider and date range with your copy and submission confirmation. The current authorization instructions say a release is not needed for records you have already supplied, but a request may concern a missing period or a different office. Ask VA to clarify the mismatch.

Can I use this register as my response to VA?

It is an organizing aid. Send the records or correctly completed forms that VA requested through the applicable official channel. The register does not replace a release, medical records, or a required response.
